How to Refer
Providers may directly refer patients to our sleep center for a sleep study consult. Our Happy Valley Sleep Center takes direct referrals for sleep studies. Please contact us for the criteria if you are interested in referring your patients directly for a sleep study. All notes will be reviewed by our Medical Director for appropriateness. When to Refer For Consult?
Reach out to us if you patients display one or more of the following symptoms:
- Acting Out Dreams
- Atrial Fibrillation or Other Arrhythmias
- Attention Deficit Disorder
- BMI of 40 or above
- Circadian Rhythm Disorders
- Commercial Driver Evaluation
- CPAP Compliance Issues
- Daytime Sleepiness
- Depression
- Diabetes
- Drowsy Driving
- Heart Failure
- History of Stroke or Transient Ischemic Attack (TIA)

Forms
We encourage all patients to complete check-in forms before their appointment.
If you are registered with InteliChart PatientPortal, you will:
- Receive a link to your check-in forms by email about 14 days before your visit
- Complete and submit your forms before your appointment
If you are not registered with InteliChart PatientPortal:
- Call your clinic to sign up for the portal
- The office will text or email you a secure link to the portal
If you are unable to complete the forms online before your appointment, please plan to arrive early to complete your forms in the waiting area before your visit.
If you have questions about check-in forms, please call the office where your appointment is scheduled.

Parking
- Enter the parking lot on NE 99th Ave using either lane.
- Take a ticket and keep it with you.
- To exit the parking lot, follow EXIT signs.
- The parking lot exits onto NE Pacific St.
Drop off and pick up
- Use the circle driveway in front of the building.
- Enter the parking lot from NE 99th Ave.
- Go forward and turn right into the circle drive.
Getting to the Office
- Go through the sliding glass doors.
- Pulmonary East clinic is on the 2nd floor.
- Enter through front door, turn right.
- You may choose to use the elevator or staircase.
- Go to 2nd floor.
- Exit the stairs/elevator and turn left.
- Look for the sign for Pulmonary, Critical Care & Sleep Medicine.
- Check in at the first desk on the right for the Pulmonary clinic.
